Requesting Your Medical Records
Please read carefully.
Medical records cannot be requested online. You must print and mail the request form located at the bottom of this page.
STEP 1:
Open the request form by clicking on the link at the bottom of the page.
STEP 2:
Once you open the pdf file, you can download and print the form. See below for instructions.
Please note, depending on which browser you're using on your computer or mobile device the screen may appear different. To download/save the document look for a symbol similar to one of the following:
To SAVE a document using your keyboard: Hold down the "Ctrl" button and the letter "S" at the same time
To PRINT a document using your keyboard: Hold down the "Ctrl" button and the letter "P" at the same time
STEP 3:
Fill out, sign, and date the form.
STEP 4:

-Requests must be mailed through the post office
-Email, online, or fax requests cannot be accepted
-Please allow up to 3 weeks for processing
-Missing information or no signature will cause delays
